[Zen Cart] Unable to activate new module during upgrade

Hello, after a server upgrade we needed to update to the latest version so I purchased Magic SEO URLs for Zen Cart v1.3.x/1.5.x 6.0
I am trying to updated this on our development server first before trying it on our production server. dev.keystonpestsolutions.com
I followed the upgrade / installation procedures carefully to the letter, ensuring that Binary mode was used for transferring all files.
After uploading the files I can activate it (as a development server), then it says "sucsessfuly activated, redirecting.. it redirects to the home page, and nothing seems to be installed. The URLS are not written, the .htaccess file is still blank, and there is no message or display telling me I am running a development version.
When I look at the activation address again dev.keystonpestsolutions.com/?InveoActivation then it shows as if the site is not registered.
Also, since this is a new version from includes/application_top.php I have removed the code: from the old version.
Verfied that .htaccess permissions: 666
cache/fancy-cache permissions: 777
Do you have any ideas why it is not saving / loading?
