[phpBB3] Installed successfully, but links are not rewritten

I've successfully installed and activated Inveo Magic SEO URLs for phpBB3, and the .htaccess file was created, but when I visit the forum homepage, or any of the subforums and topics, the URLs remain their old dynamic selves (viewforum.php?f=2, etc).
Quite a few PHP debugging notices appear at the top, such as these:

Are these errors related to the fact that links are not rewritten, or should I be looking at something else?
This is on a server running CentOS 5.5 with Apache 2.2.3 and PHP 5.3.6. I'd rather not share the forum URL publicly; it is live, but all IPs other than my own are blocked as long as it's in development.

These error messages are displayed if you try to install Magic SEO URLs for phpBB 2.x to phpBB3.
Please make sure you are using correct phpBB version or purchase Magic SEO URLs for phpBB3 if you are using phpBB3.

According to the order confirmation, I purchased "Magic SEO URLs for phpBB 3.0", and the file I downloaded and from which I installed is "Inveo_Magic_SEO_URLs_for_phpBB_3.0.03". I should note that I am running phpBB 3.0.8 -- is that version compatible with the package I ordered?

Ah, now I see the difference. There are two seemingly identical products on your site, "Magic SEO URLs for phpBB 3.0" and "Magic SEO URLs for phpBB3 3.0". I guess I did not notice the difference when I ordered. Can this order be changed from phpBB 3.0 to phpBB3 3.0? If not, please issue a refund and I'll order again.

Due to technical limitations this can not be change but we can refund your order and then you can make an another one.

Thank you. I appended a message to the order from within My Account / Order History.
Has the Magic SEO URLs installation made any changes to my database that I should revert before installing the correct version?

No.
Magic SEO URLs do not change any files or anything in database.
